To help make your first visit to our practice as smooth as possible, please read the information below. You can also save some time by printing out and completing the new patient forms in advance of your appointment. We look forward to meeting with you and providing you with podiatric care.
What To Expect
Allow ample time to fill out and review forms (if you did not complete the forms prior to your visit), for your consultation with the podiatrist, and be examined. Be sure to bring any and all treatment records, X-rays, MRI reports, etc. to your appointment to ensure that Dr. Stephen Chapman and Dr. R. Gregory Walters has all of the needed information to provide the best possible care for you.
